Schedule a consultationBefore you come in
We collect a short medical history, an insurance form, and a HIPAA acknowledgment before treatment starts. You can finish the paperwork at home or in the office — whichever fits your schedule.
[FILL IN: link to the online intake (Weave forms or PDF packet) once the practice confirms which channel is current. If neither is in place, the front desk hands paperwork on arrival and the visit starts about ten minutes later than the scheduled time.]
Bilingual support is available throughout — front desk through chair-side, English and Spanish. If a translator helps, tell the front desk when booking and we will plan the visit accordingly.
Call the front desk · 661-323-5910What the first visit looks like
A consultation is unhurried and conversational. Here is the rhythm of it, in order.
The front desk verifies insurance, confirms paperwork, and answers anything you have not been able to ask yet. If you have a referral or prior records, this is when we pick them up.
Dr. Mendivil examines the bite, jaw, and surrounding teeth in a private treatment room. For orthodontic cases we take panoramic and intraoral x-rays; for Invisalign cases we capture a 3D intraoral scan instead of putty impressions.
Findings, treatment options, timeline, and what your insurance covers — said in plain English, in the same room, before any treatment starts. You leave with a written estimate.
Most patients decide later, at home. The front desk can hold a tentative slot; nothing is committed until you say so.
Bring with you
Most of this you already have. The point is fewer trips back to the car, and a faster start at the front desk.

Driver license or any government-issued ID. For minors, the accompanying parent or guardian brings theirs.
Including dosage. Mention recent surgeries, pregnancy, or anything currently being treated by another doctor.
If your general dentist took panoramic or intraoral x-rays in the last twelve months, ask them to email a copy. Saves a duplicate set.
Required at the first visit. [FILL IN: confirm whether minors may attend follow-up adjustment visits unaccompanied — current site does not specify.]
Between visits
Most of orthodontics happens between appointments. The handful of things worth calling about, with the practical answer.
Call the office first thing. We hold same-day slots for orthodontic repairs when the situation warrants it.
Same as above — call right away. We triage at the front desk and find the soonest available slot.
A 24-hour notice keeps the chair time open for someone else who needs it. Call or message us through Weave.
[FILL IN: confirm after-hours emergency line and voicemail instructions — current site has no published after-hours number.] If you cannot reach the office and the situation is urgent, your nearest emergency room can handle dental trauma.

If anything on this page raised a question, the front desk would rather hear it before the visit than during. Give us a call.
